This compound is a highly toxic product if ingested. Due to its sweet taste, there are many reports of accidental ingestion particularly of antifreeze products, by children and pets. Intoxication symptoms can be divided in 3 stages:
- Stage 1 – up to 12 hours after ingestion: Dizziness, incoordination of muscle movements, headaches and confusion, followed by nausea and vomiting
- Stage 2 – 12 to 36 hours after ingestion: Increased heart rate, high blood pressure and metabolic acidosis as a result of accumulation of glycolic and oxalic acids in the bloodstream. If not treated in time, this could be fatal.
- Stage 3 – 24 to 72 hours after ingestion: high levels of potassium in the blood, acute tubular necrosis, blood in the urine, lower back pain and acute kidney failure.
The most accurate way of diagnosis is by assessing the level of ethanediol directly in the blood. If doctors do not have the facilities to perform this test, diagnosis can be done by the presence of oxalate crystals in the urine. Treatment involves the use of antidotes, such as ethanol or fomepizole, which block the enzyme capable of metabolising ethanediol. This works because it’s not the ethanediol itself that is toxic, but the products of its degradation. If treatment is initiated in time, most patients can make a full recovery.
